Sugioka¡¯s Transtrochanteric Rotational Osteotomy in the Treatment of Osteonecrosis of the Femoral Head

Abstract
Purpose: The purpose of this study was to evaluate the results of 19 cases of Sugioka¡¯s osteotomy for nonsteroid induced osteonecrosis of the femoral head in young, active patients.

Materials and Method: Rotational osteotomy was performed on 22 hips(20 patients) with extensive nonsteroid osteonecrosis from April, 1994 to October, 1999, Nineteen hips of 18 patients were avilable for clinical and radiological follow-up at an average of 44 months(36~85 months). The average age of the patients at the time of the index procedure was 34.5 years(23~42 years). The anterior rotation applied to the femoral head was 70¡Æ to 90¡Æ, while 5¡Æ to 10¡Æ intentional varus position was added in accordance with the extent of nectotic area in all hips. The results were considered successful if there was no radiologic failure(progression to necrosis, collapse) or clinical failure(the need for total hip arthroplasty)

Results: Fifteen(78%) of 19 hips exhibited successful results according to clinical and radiographic criteria. Four cases of progressive varus deformity occurred after the procedures, 3 of which, along with one other, were converted to
total hip arthroplasty because of head collapse and infection after valgus osteotomy in one hip.

Conclusions: Our results demonstrated that Sugioka¡¯s osteotomy without vascular injury is a dependable procedure in the treatment of nonsteroid osteonecrosis of the femoral head at stage ¥±or ¥², especially in young patients. Nevertheless, concerns remain regarding the fixation method after trochanteric osteotomy.
